Everything You’ll Need

To make your enrollment process as easy as possible, we have TWO ways for you to enroll:

Option 1: Call Center Enrollment within 30 days of your hire date or qualifying life event (QLE).

The Benefits Service Center through the Call Center allows you to speak with a Benefits Counselor 1-on-1 at 877.223.1869. Monday through Friday from 9 AM to 8 PM EST (closed on weekends and holidays).


If you are experiencing issues uploading a document or processing your EOI, please email your documents in a PDF format to [email protected]. Please include your full name and the reason for your email.

Option 2: Self-Enroll Online within 30 days of your hire date or qualifying life event (QLE).

Self-enroll 24/7 through UKG. Click here to review your benefits and make any changes.
Once logged in, select Benefits> Manage My Benefits> Click on “Get Started” to begin making your benefit selections. Remember to “Checkout” to save your selections. We recommend saving/downloading a copy of the benefit confirmation for your records. Access your Benefit Confirmation Statement at any time!

Remember: You must update your contact information through UKG, including your home address, primary and alternate email, cellphone number, and other demographic information, before completing your enrollment elections in Plansource and uploading documentation when adding or removing new dependents during the Enrollment.

Step 3: After Enrollment Pending Tasks (if applicable)

After completing your Enrollment process through PlanSource, you must submit NEWdependent’s documentation through the system or email a document or proof of QLE in a PDF format to [email protected] within 30 days. Please include your full name and the reason for your email.

Vol Life coverage – Evidence of Insurability Online Form: Once the EOI is submitted through the benefits portal, PlanSource directly from UKG, click here> by clicking Benefits > Manage My Benefits. Click on “Get Started” to see your pending To-Do list > and select the EOI option to begin your online application through the Standard. It will be reviewed by the Standard underwriting team, and if additional information is needed, they will contact you directly. Once a decision has been made, a letter will be mailed to you. NOTE: You have 30 days to complete the health questions for you or your spouse. If the EOI form is not submitted after completing your enrollment process, you will only receive the Guaranteed Issued amount, and the system will automatically close your request. Your next opportunity to enroll will be during the next open enrollment period.

đź”»Name changes must be processed through UKG, and proof of documentation should be emailed to [email protected] within 30 days of enrollment!


  • Distribution of Form 1095-C. Employers are no longer required to automatically mail the Form 1095-C to individuals. The forms are available electronically in PlanSource: Log into UKGPro > Myself > Manage My Benefits to access the PlanSource portal / Navigate to “Documents” on the left toolbar of your portal / Select “Tax Documents” / Follow the steps to download the 1095-C electronically.
  • Electronic access to the forms will remain available in PlanSource throughout the year for your convenience.
